change of ownership request must be submitted in writing.
Please include a recorded copy of the document, your name, owner
number, address and a daytime phone number.
For
a change of ownership due to the death of an owner, a
copy of the owner's death certificate must also be included.
Name Change:
To change the
name of your ownership, please submit a copy of your Marriage
Certificate, Divorce Decree, or Court Order evidencing the
requested change.
